PT Asthma Community Health Care Worker

The Asthma Community Health Worker (CHW) will work closely with the DOT House Pedi asthma team, acting as the liaison between DH and the families of pediatric asthma patients. Responsibilities include making home visits to conducting environmental assessments. Sharing important information obtained from contact with families with the DH pediatric asthma team. The CHW will also work closely with the Boston Urban Asthma Coalition (BUAC) around the illness’s issues involving the patient’s school or daycare.
